Senior Farmers Market Nutrition Program

The Senior Farmers Market Nutrition Program (SFMNP) provides resources in the form of an EBT card to purchase fresh and nutritious fruits, vegetables, edible herbs, and honey and provide nutrition education to eligible senior residents in the state of Washington.

Please use the form below to apply for the Farmers Market EBT card.

  • All entries will be screened for accuracy.
  • All fields are required to apply.
  • Paper copies of the affidavit and proxy forms are also available for pickup, if needed, at The Diner Vancouver, 5303 E Mill Plain Blvd, Vancouver, WA 98661 , Wed-Sun 10 am – 3 pm.  Completed forms should be mailed to Meals on Wheels People, Attn: SFMNP,  1681 C St., Washougal, WA  98671

Applicants:

Participants who attest and qualify for the Senior Farmers Market Nutrition Program will be able to pick up their benefit card at one of the designated locations and times. Cards not picked up in person will be mailed to participants starting June 10. Times and locations are subject to change. Check this website for the most up-to-date information.

If you have applied for a new EBT card after June 5, you will receive your card by mail within 10 to 12 business days.

    Applicant Information












    The Senior Farmers Market Nutrition Program (SFMNP) provides fresh fruit and vegetables to lower-income seniors with the goal of improving their health and nutritional status. It also supports local farming by increasing the use of farmers markets and roadside stands.

    To be eligible for the SFMNP, you must meet all of the following:

    1. You must be 60 years old or older (or 55+ if you are Native American / Alaska Native)

    2. Your income must be below 185% of Federal Poverty Level. That means:
    - below $2,322 monthly income for 1 person
    - below $3,151 monthly income for 2 people
    - For larger households, add $829 for each additional person

    3. You must be a resident of Clark County, Washington
